A rogue ninja slipped in through the leaf gate and mortally injured Shiho Uzumaki in his sleep. Anaki Uchiha used his medical skills to revive the Hokage, but when he said "Lord Hokage, are you all right?" Shiho responded "Who are you?" so Anaki has to take over as Hokage.
